California Employers Only
California employers are required to file a DWC1 form within 24 hours of a work-related injury. For additional copies of DWC1 forms, call Uniform at 800-382-2424. For all other informational pamphlets, please call California WC Institute directly at 510-251-9470.
Note: Report claims promptly. A delay in reporting a work-related injury or any other type of claim can directly increase the cost and lengthen the time for investigation.
